Appears to be rooted. Grows to above the water surface. There is algae mixed with it as well.

Lithobates catesbeianus or Rana catesbeiana ?
For the weed, take a single strand, place it on a white background and take a good, clear close up picture. I will float/suspend a strand in a clear glass baking dish with water in it and sheet of white paper under it.
I could guess at an ID, but that's all it would be. My guess would be Myriophyllum spicatum or a species of Potamogeton.
