Yes it could be any of several reasons. Good to point them all out. I like the pics you added above. He needs to look further into possibilities and causes. More info on stocking times and species , lake size , mgt efforts and timing would help us narrow the possibilities. Also could send off a fish or two to AU for diagnosis if warranted.

I don't think we know all the possible deficiencies resulting from high levels of pelleted feed for long periods. Vit C yes , others possibly. That is why on carnivorous fish (LMB , YP , HSB , BG and others) I want fish meal/oil as it most closely resembles (amino acids/proteins) their natural diet. There is a lot unknown about fish diets/nutrition and its effects. Having said that however pellets beat the heck out of having starving fish.
